Solomon Medical is hiring a full-time physician for our Monterey County post-acute care programs serving skilled nursing and rehabilitation facilities in Salinas and Monterey. This role is a strong fit for an Internal Medicine, Family Medicine, Geriatrics or Hospitalist physician who enjoys complex patient care but wants a more flexible and sustainable schedule than many hospital-based roles.
Physicians typically round in skilled nursing and rehabilitation facilities, manage post-acute and long-term care patients, collaborate with facility teams and remain available for clinical follow-up during scheduled coverage hours. The role offers meaningful autonomy, steady clinical variety and the ability to build long-term relationships with patients, families, and care teams.
You will not be on your own. Solomon Medical provides medical leadership, APP collaboration and a dedicated operations team that helps keep workflows organized and manageable. Providers care for a broad range of medical and rehabilitation cases and work closely with interdisciplinary care teams to support continuity of care and patient outcomes.

Job Responsibilities
Perform new admissions, follow-up visits, and routine patient evaluations
Manage acute and chronic medical conditions in the PALTC setting
Develop and coordinate treatment plans with interdisciplinary care teams
Communicate with patients, families, facility staff and Solomon Medical team members
Complete timely documentation and billing within the EMR
Collaborate with APPs, nursing leadership, therapists and facility teams
Support continuity of care, quality initiatives and facility protocols
Qualifications
Board Certified in Internal Medicine, Family Medicine, Geriatrics, or related primary care specialty
Active California medical license
DEA license
Strong clinical judgment and communication skills
Comfortable managing medically complex and chronic care patients
Organized, collaborative, and patient-centered
SNF, post-acute care, hospital medicine, geriatrics, or primary care experience preferred
Working Hours
Full-time opportunity with part-time flexibility for the right fit
No overnights/weekends
