Company Information
Solomon Medical is a physician-led practice providing primary care and behavioral health services in post acute and long-term care (PALTC) settings Central and Northern California.
We care for patients during an important stage of recovery, often after a hospitalization surgery or serious illness. Our practice is built on strong relationships with the facilities we serve and a collaborative approach to patient care.
In addition to clinical services, Solomon Medical also partners with facilities through consulting and administrative support services focused on improving communication, coordination and quality of care.
We support our physicians and advanced practice providers (APPs) with responsive operations, administrative support and practical workflows so providers can spend more time caring for patients and less time dealing with unnecessary administrative burden.
