Geriatrician | Internal Medicine-Geriatrics | Help Shape a New Community Care Program

St. John’s Community Health is seeking an Internal Medicine-trained Geriatrician to help expand how the organization cares for older adults across Los Angeles and surrounding communities.

This is more than a traditional outpatient geriatrics role. The physician will provide comprehensive care for older adults while also helping shape the longer-term direction of St. John’s geriatric community care program.

The model is designed around the realities of aging — not only chronic disease, but also cognition, function, medication burden, caregiver needs, mobility, social circumstances, and access to community resources. The Geriatrician will work closely with interdisciplinary teams and help coordinate care across primary care, behavioral health, specialty care, pharmacy, social services, caregivers, and community partners.

A distinguishing part of the role is the opportunity to provide care both inside and outside the clinic. In addition to seeing patients within St. John’s clinics, the Geriatrician may support mobile health efforts, home visits, and partnerships with community- and faith-based organizations through health education, screenings, wellness monitoring, and care coordination.

As St. John’s first Geriatrician, this physician will also have the opportunity to bring thought leadership to the development of the organization’s long-term geriatric community care strategy and help shape how comprehensive care for older adults is delivered across the system.
