Physician - Internal Medicine / Family Medicine / GeriatricsJob Details:
- Provide high quality, patient-centric, cost-effective inpatient level of care in the home setting
- Work within a team environment alongside Physicians, NPs, and RNs in a virtual Command Center
- Deliver integrated care in coordination with primary care providers, specialists, and ancillary departments
- Utilize state-of-the-art equipment available at the Command Center and in patients' homes
- Support patients with access to home infusion, meals and nutrition consults, pharmacy and medical management, mobile imaging, laboratory services, and DME and oxygen
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics required
- Committed to providing high quality, evidence-based medicine to a diverse patient population
- Excellent clinical and communication skills required
- Demonstrated ability to work effectively in a team environment required
