A health services network in Rockland County, New York is currently seeking a licensed Nurse Practitioner to provide Primary Care to their local LGBTQ+ community.
About the Opportunity:
- Schedule: Monday to Friday
- Specialty: Primary Care
Responsibilities:
- Provide direct care and consultations to patients
- Serve as a consistent Care Coordinator through the continuum of care
- Assess the physical, psychological, spiritual, and social needs of the patient
- Direct patients to the next appropriate service and track their progress
- Assure that all patients receive appropriate evaluation, diagnosis, treatment, medical screening, and medical evaluation whenever indicated
- Perform other duties, as needed
Qualifications:
NYS Nurse Practitioner (NP) license
