Advance Community Healthcare, is expanding our services and are recruiting for a Family Medicine Physician who will provide services to our new Wake Forest and/or Louisburg, NC locations working a split schedule between locations.
We are seeking a compassionate and dedicated Family Medicine Physician to provide comprehensive, full-scope primary care to patients of all ages, including pediatrics and adults. This role is ideal for a physician who values continuity of care and enjoys building long-term relationships with individuals and families across generations.
We welcome candidates with at least one year of experience, as well as recent residency graduates who are passionate about delivering patient-centered care in a community health setting.

Job Responsibilities
• Diagnose and treat acute and chronic medical conditions across the lifespan.
• Perform routine check-ups, well-child visits, and preventive health screenings.
• Manage chronic diseases such as asthma, diabetes, hypertension, and obesity.
• Deliver age-appropriate care, including immunizations and developmental screening.
• Collaborate with a multidisciplinary care team including nurses, medical assistants, behavioral health, and care coordinators.
• Educate patients and families on health maintenance, disease prevention, and treatment plans
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and patient-centered care programs.
Qualifications
• Board Certified or Board Eligible in Family Medicine.
• Minimum of 1 year of clinical experience preferred, but new graduates are encouraged to apply.
• Active, unrestricted North Carolina medical license (or ability to obtain.
• Active BLS (Basic Life Support) certification.
• Active DEA license
• Experience or comfort treating both pediatric and adult populations
• Strong commitment to community health and serving diverse/underserved populations
