**Opportunity:** PHYSICIAN - Family Medicine with Clinical Leadership: This is a full-time opportunity for a Family Medicine physician who enjoys diverse practice elements including primary care, emergency medicine, hospital medicine, and nursing home care. The position includes leadership development with dedicated time for mentoring and consulting with team members.
**Job Details:**
- Full-time, 1.0 FTE position with leadership development opportunities
- Work as a leader of a Care Team, collaborating with other healthcare members
- ER call is negotiable
- Practice includes variety across primary care, emergency medicine, hospital medicine, and nursing home care
- Partner with and lead a team of 3 experienced Advanced Practice Clinicians
- Utilize established care processes to achieve patient care objectives
- Provide continuity of care for patients and families
- Access to onsite services including Diabetes Education, Dietitian, Child Behavioral Health, Therapy Services, Medication Management, and more
- BC/BE in Family Medicine required
