Job description
St. John’s Community Health is seeking its first Geriatrician to help expand how the organization cares for older adults across Los Angeles and surrounding communities.
This is more than a traditional clinic-based geriatrics role. The physician will provide comprehensive care for older adults while also helping shape the longer-term direction of St. John’s geriatric community care program.
The model is designed around the realities of aging — not only chronic disease, but also cognition, function, medication burden, caregiver needs, mobility, social circumstances, and access to community resources. The Geriatrician will work closely with interdisciplinary teams and help connect care across primary care, behavioral health, specialty care, pharmacy, social services, caregivers, and community partners.
A distinguishing part of the role is the opportunity to provide care beyond the traditional clinic setting. In addition to seeing patients within St. John’s clinics, the Geriatrician may support mobile health efforts, home visits, and partnerships with community- and faith-based organizations through health education, screenings, wellness monitoring, and care coordination.

Job Responsibilities
Manage acute and chronic medical conditions affecting aging populations
Conduct geriatric, cognitive, functional, and fall-risk assessments
Evaluate and manage dementia, depression, frailty, mobility concerns, and other age-related conditions
Perform medication reconciliation and address polypharmacy risks
Develop individualized care plans focused on function, safety, independence, and quality of life
Support advance care planning and goals-of-care conversations
Coordinate care with specialists, behavioral health providers, pharmacists, social workers, case managers, caregivers, and community resources
Participate in interdisciplinary case discussions and quality-improvement efforts
Provide education and support to patients, families, and caregivers
Participate in community outreach, including opportunities for mobile health, home visits, and work with community- and faith-based organizations
Supervise Advanced Practice Providers as assigned
Qualifications
Completion of an accredited Internal Medicine or Family Medicine residency
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Geriatric Medicine fellowship preferred
Board Certified or Board Eligible in Geriatric Medicine preferred
Active California Medical License
DEA and current BLS certification
Experience caring for older adults and medically complex patients
Strong interest in community-based medicine, health equity, and interdisciplinary care
Comfortable providing care outside a traditional clinic environment, including home and community settings
