Healthcare Organization (HCO)
A contracted vendor for the New York Independent Assessor Program (NYIAP) in partnership with the New York State Department of Health.
Is responsible for conducting conflict-free Medicaid assessments for individuals seeking personal care services and community-based long-term services and supports.
- Nurse Assessors complete the Community Health Assessment (CHA).
- Independent Practitioners (NPs, PAs, MDs) perform the clinical appointment to determine whether the individual is medically stable to receive services in the community.
- Assessment findings are used by Local Departments of Social Services, Medicaid Managed Care Organizations, and Managed Long-Term Care plans to determine eligibility and care planning.
This role directly supports the NYIAP program through Face-to-Face home clinical evaluations for elderly Medicaid members.
Position
Nurse Practitioner or Physician Assistant – Face-to-Face Home Medical Assessments
- Ongoing locums assignment, start ASAP
- Seneca Falls: 1 provider, 9 hrs/week
- Niagara Falls: 2 providers at 9 hrs each OR 1 provider for all 18 hrs
- Elderly Medicaid population only
- 1-hour assessments; 45-minute pay for no-shows (2–5% rate)
- Paid for all drive time (no mileage reimbursement)
- No follow-ups; paid only for hours worked (no lunch pay)
- Provider may choose any day of the week; cannot exceed assigned weekly hours initially
- Shift windows:
- Monday–Friday: 7:45a–7p
- Saturday: 9:15a–6p
- Charting:
- Practitioner’s Order (PO) form (~15 min)
- Review Community Health Assessment (CHA) (~30 min)
- All documentation completed via secure platform
Compensation
- $80–$85/hr
- Paid for all drive time
- No mileage reimbursement
- No set contract length; extensions available
Requirements
- NP: 2 years’ experience + 3,600 hours as an NP; national certification required
- PA: Experience preferred (HCO standard applies)
- Active NY State License
- COVID vaccination required (flu vaccine not required)
- Provider must have personal Medicaid enrollment (OPRA number required)
- Must not be registered as a Medicaid biller under their own certification
- Cannot work for an insurance provider (conflict of interest)
