Opportunity:

Advanced Practitioners - Primary Care

Job Details:

  • Full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA-C) opportunity in a multi-provider, outpatient primary care practice
  • Serving patients age 12 and older across two locations in Plainfield and Putnam, CT
  • Schedule: 4 days per week (3 days in Plainfield and 1 day in Putnam); weekdays only with flexible schedule options available
  • No call required
  • Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
  • Develop plans for further evaluation of health needs, including referrals to specialists and other health team members
  • Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
  • Work under established guidelines in collaboration and/or supervision with practice physicians
  • Dedicated support through the Office of Advanced Practice for Nurse Practitioners and Physician Assistants
  • Unrestricted State of Connecticut medical license required (or eligible)
  • APRN (Family Nurse Practitioner) or PA training required
  • Two or more years of post-graduate experience in Primary Care or a related field seeing both pediatric and adult patients preferred
