We are seeking an experienced Nurse Practitioner to join our collaborative primary care team. In this role, you will evaluate and treat patients with acute and chronic health concerns, build and manage your own patient panel, and collaborate with physicians and specialists to ensure exceptional patient outcomes. Experience with electronic medical records is preferred, and training on our EMR platform will be provided.

Qualifications
- Adult or Family Nurse Practitioner Certification
- Active New York license or ability to obtain
- Minimum of 2 years of Nurse Practitioner experience
