We are seeking a full-time Nurse Practitioner to join our skilled nursing facility (SNF) care team in the Fall River, Massachusetts area. This role offers exceptional flexibility, strong team support, and meaningful work caring for post-acute and long-term care patients.

This position is ideal for Nurse Practitioners seeking work–life balance, including those with young families or personal schedule needs.

Responsibilities

  • Provide medical care to patients in local skilled nursing facilities
  • Conduct rounds independently while collaborating with onsite medical directors
  • Develop and manage individualized patient care plans
  • Support coordinated care across hospital, SNF, and home settings
  • Focus on reducing hospital readmissions and improving outcomes
  • Work closely with nurses, therapists, pharmacists, administrators, case managers, and physicians

Qualifications

  • Active/Pending Nurse Practitioner license in the Commonwealth of Massachusetts
  • Strong organizational skills and ability to work independently
  • Excellent problem-solving abilities and clinical judgment
  • Compassionate communication with patients and families
  • Ability to collaborate effectively within an interdisciplinary team
  • Portuguese language skills are a plus (not required)
