Job description
We are seeking a compassionate and experienced Client Physician (MD/DO) to provide specialized Client services across home health, nursing homes, and assisted living facilities. This field-based position focuses on evaluating, diagnosing, and treating patients with chronic wounds, pressure injuries, surgical wounds, diabetic ulcers, and other complex skin conditions.
The ideal candidate will deliver high-quality, patient-centered care while collaborating with interdisciplinary healthcare teams to improve healing outcomes. This role also includes medical documentation, care coordination, supervision of clinical staff, and ensuring compliance with regulatory and organizational standards.

Job Responsibilities
Perform comprehensive wound assessments and develop individualized treatment plans.
Evaluate, diagnose, and manage chronic wounds, pressure injuries, surgical wounds, diabetic ulcers, and other complex skin conditions.
Perform advanced Client procedures, debridement, and other evidence-based treatment interventions as appropriate.
Monitor wound healing progress and modify treatment plans based on patient response.
Coordinate patient care with primary care providers, specialists, nurses, therapists, and caregivers.
Supervise and provide clinical guidance to nurses and medical assistants.
Educate patients, families, and caregivers on wound prevention, treatment, and self-care techniques.
Maintain accurate and timely documentation within the Electronic Medical Record (EMR) system.
Ensure compliance with HIPAA, infection control standards, and organizational policies.
Participate in interdisciplinary team meetings, case conferences, and daily clinical updates.
Maintain accurate documentation of patient visits, communications, and treatment plans.
Travel to patients' homes, nursing homes, and assisted living facilities to provide onsite care.
Support staff orientation, education, and ongoing clinical training.
Perform additional duties as assigned by the Chief Medical Officer.
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
Active, unrestricted MD/DO license in the practicing state.
Minimum of 2 years of physician experience.
Recent direct patient care experience preferred.
Willingness to obtain Client Certification within six months of hire.
Valid driver's license, reliable transportation, and current auto liability insurance.
Ability to travel throughout the assigned service area (100% travel required).
Preferred Qualifications:
Board Certification in a relevant specialty.
Previous experience in Client, Home Health, Long-Term Care, Skilled Nursing, or Geriatric Medicine.
Experience managing diabetic foot ulcers, pressure injuries, venous ulcers, arterial ulcers, and surgical wounds.
Knowledge of advanced wound healing technologies and infection management.
Working Hours
Field-based position requiring daily travel to patient homes, nursing homes, and assisted living facilities.
Ability to stand, walk, kneel, bend, squat, lift, and assist patients as needed.
Ability to lift or assist in transferring patients weighing up to 100 pounds.
Exposure to infectious materials and biohazards while following standard infection control protocols.
Annual TB testing and compliance with state health requirements.
