Job description Breakdown of Clinic Days/Sessions and/or OR Days: ½ day on Monday afternoon, all day Thursday
Schedule of Days/Times Needed To Work/ Shift Hours: Monday 1/2 day, Tuesday OR all day, Thursday Clinic all day

Procedures Provider Will Be REQUIRED To Perform: Surgical Cases


Anticipated Procedures/Surgeries per Week: Work all day on Tuesday


Patient Load: 6 per session (half day) minimum


Board Status Requirement (BC/BE): Must be Board Certified


Certifications Required? BLS, ACLS, PALS?? BLS, ACLS, PALS


Is there additional support staff? Yes


Is the provider working solo or with other providers? If so, whom? With APP


Are there mid levels? If yes, is the locums provider responsible for mid levels? Yes, 1 APP


Trauma Level of Work LocationL Level 1


EMR System? EPIC


Is the Clinic EMR Different From the Hospital EMR? No

Additional Details:
• Will they need to be fellowship trained in pediatric urology? Fellowship is required and experience is much preferred
• Can you clarify what the call will look like? Weeknights? Weekends?? Typically I’ve asked a locum to be on call for the entire week when on. We will not be asking for someone to come for weeks at a time, but a rotation of weeks.
• Which days are the 2.5 clinic days? Hours? To clarify, it’s 1 day of surgery, 1.5 days of clinic as noted on the form. Monday 1/2, Thursday all day for clinic (8-5), OR on Tuesday all day.
• Which day is OR day and what are those hours? Hours will depend on number of cases, but block time is scheduled from 7:30 to 5
