Requirements

• MD or DO
• Board Certified or Truly Board Eligible in Plastic Surgery
  • Must obtain board certification within 5 years
  • No exceptions
• Active, unrestricted Iowa medical license
• DEA
• BLS certification and maintain current certification
• Clean malpractice and license history preferred
• J-1 and H-1B visa sponsorship is not available
• Self-Query NPDB pulled within 30 days of submission
