Job Description

A Sacramento based client is seeking a motivated and compassionate family medicine or internal physician to join a collaborative, outpatient practice. An ideal candidate should value practicing evidence-based medicine and have an addiction medicine specialty.

Required Qualifications

  • CA medical license as an MD or DO
  • Completion of an approved residency program
  • Board certification in internal medicine, family medicine, or psychiatry (APP/MD)
  • CA DEA certificate before start date
  • CPR certification
  • Spanish fluency (preferred)
  • Board eligible/certified in Addiction Medicine (preferred)
  • Experience with FQHC (preferred)

Responsibilities

  • Provides outpatient addiction medicine services.
  • Provide direct clinical care in addiction medicine.
  • Develop outcomes-based treatment plans of care that address mental health, addictions, functional impairment, and co-occurring addictive and/or medical disorders
  • Engage the patient and family to develop treatment plans that address identified goals
  • Provide a minimum of 30 patient contact hours per week
  • Champion a model of integrated care that focuses on the full spectrum of patient health through close collaboration with multi-disciplinary teams
  • Consult with primary care providers to assist with ongoing management of the behavioral health needs of their patients
  • Develop and lead staff education in psychiatric and/or addictive issues
  • Direct supervision of mid-levels
  • Participate in relevant and ongoing professional training
  • Responsible for accurate and timely documentation necessary for patient care and billing.
  • Responsible for assuring quality medical care delivered to all patients.
  • Responsible for understanding and maintenance of Quality Improvement Program indicators as directed by the Leadership for the medical department.
  • Responsible for understanding and complying with the Standards of Care and policies for the Medical Clinic.
  • Responsible for understanding and complying with the implementation of medical clinic policies to ensure appropriate medical follow-up of patients and medical specialty referrals.
  • Understands and follows medical operations in compliance with applicable state and local laws, regulations and policies.
  • Understands and monitors systems for compliance as necessary. Provides necessary and timely communication to Medical Leadership when issues of compliance arise, and complies with a corrective strategy.
  • Understands and follows policies and procedures relative to medical provider scheduling, schedule changes, template development, etc., coordinating with efforts with the Practice Manager. Promptly communicates problems and issues in these areas to the Medical Leadership.
  • Assists other medical and general staff, in developing and designing new clinical services, as appropriate to One Community Health patient needs.
  • Responsible for attending all appropriate meetings as directed by the Medical Leadership.
  • The Addiction Clinician is responsible for quality medical services to One Community Health patients. They are responsible for compliance with applicable laws, regulations and policies pertaining to medical practice, clinic operations and licensing.
