Job Responsibilities
• Conduct health screenings and risk assessments
• Manage chronic disease prevention initiatives
• Educate patients on lifestyle modifications and disease prevention
• Collaborate with healthcare teams to improve population health outcomes
Qualifications
• Active Oregon Medical License required (or willingness to obtain)
• Experience in public health or primary care preferred
• COVID-19 and Flu vaccination may be required
Working Hours
