Physiatrist (PM&R) MD/DO, Subacute Rehab, Part-Time, Nassau County, NY

Medley Health is seeking a compassionate Physiatrist to provide on-site physiatry and rehabilitation services in subacute rehab facilities. You’ll practice independently with strong administrative support, working alongside interdisciplinary teams to help patients regain function, improve mobility and manage pain.

What You’ll Do

• Perform comprehensive physiatry evaluations and follow-up visits in subacute rehab settings
• Diagnose and manage musculoskeletal, neurologic and functional limitations
• Develop individualized treatment plans focused on functional restoration, mobility and pain management
• Prescribe and manage medications, including controlled substances, as part of a rehabilitation and pain management plan
• Administer therapeutic injections, including joint and trigger-point injections
• Coordinate plans of care with PT, OT and ST teams
• Document assessments and progress notes accurately and on time
• Communicate recommendations to facility clinical teams and support care transitions

Qualifications

• MD or DO; Board Certified or Board Eligible in PM&R preferred
• Active New York medical license
• Active DEA registration
• Experience in subacute rehab, geriatrics, rehab medicine or a related setting
• Strong clinical judgment, documentation and communication skills
