o Does Provider require hospital/surgery center privileges? No
o Are temporary privileges available? No
o Patients Per Shift: TBD
o Beds in Department: 33
o Admissions Required? No
o Is Hospital Stroke Certified? No
o Rounding Patients: Both
Does your facility require physicians to be board certified? Yes
Minimum board certification: Certified
Board certification must be for Urology - Pediatric Urology? No
Estimated credentialing timeframe (Days):120 days
ACLS: Yes
BLS: Yes

Other: ERCP Required
