Requirements:

• Must be Board certified in hand surgery or a Truly board eligible - [REQUIRED]
• Must have completed an Orthopedic residency - [REQUIRED]
• Must have fellowship training in Hand & Upper Extremity Surgery - [REQUIRED]
• Have an active license in both Iowa and Illinois - [Preferrred. but will review new licensing candidates too!]
• Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - [REQUIRED]
• Open to both graduating fellows and experienced hand surgeons
• Does the provider require visa support – H-1B cap exempt (Note in Additional Submission Details) – [REQUIRED]
• Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- [REQUIRED]
• Board Certified
