✔ MD or DO
✔ Fellowship Trained in Orthopedic Sports Medicine
✔ ABOS Board Certified or Board Eligible
✔ Eligible for Iowa & Illinois Medical Licenses
✔ Strong Arthroscopic & Open Sports Surgery Skills
✔ Active DEA
✔ Current BLS Certification
✔ Clean Malpractice & License History Preferred
