- Located in Crown Point, IN; Chicago 45m
Overview
* The health center is seeking a General Dentist for its dental clinic (full time)
* Provide dental services within the Patient Centered Medical Home (PCMH) model
* Perform clinical and radiographic evaluations to diagnose and treat diseases of the oral cavity
* Provide education on preventive and restorative dentistry and oral hygiene
* Document and follow treatment plans, and follow all dental clinic protocols
* Must meet the health center's credentialing requirements
Qualifications
* DDS or DMD, graduate of an accredited Dentistry program
* Indiana dental license
* Two years of experience in a healthcare setting preferred
