Temporary privileges available
November 30th and Ongoing need
7-12 follow up 12-15 active treatment per shift average
Ohio or IMLC license required to apply
Board Certified
30 day credentialing wait
Level 3 trauma facility
Orientation Date: *We will need to schedule an orientation shift prior to 12/27. Ideally the week before the assignment.
Coverage Dates: 11/30 ongoing--8 hour clinical days with no call. Seeking 4-5 days of coverage per week. No weekends, No call
Start Time: 7:30a End Time: 4:00p 7-10 pt per day depending on new or follow up, add Ons, and urgent visits • 1 day per week is reserved for OTV (On treatment visits). STAFF 1 CLINICAL MANAGER, RTT 1.6 RADONCS (0.6 vacancy) 1.6 FT RN, OCN, .6 MST 2 FT PHYSICIST (1 vacancy) 2 FT CMD 5.4 STAFF RTT ACR Accredited ACoS Accredited HFAP Accredited NSABP Accredited with the Breast Center QOPI Certified with Medical Oncology HOSPITAL EPIC EMR ARIA RO TREATMENT EMR
