- Monday-Friday 8am-4:30pm
- Provider Type – MD/DO
- Specialty: Radiation Oncology
- FTE needed: 1.0
- Daily Shift Standard (8hr or 12hr): 8hr
- Expected duration of assignment: 6 months (26 weeks)

- Schedule /Shift hours: Monday-Friday 8am-4:30pm

- Average patients per day: Variable. 15 on Treatment daily/2-5 consults a week/5-10 follow-ups week
- Required Procedures: No procedures outside of radiation oncologist-consults, simulations, follow-ups in clinic operations. No surgery etc.
- Required clinical activity in the last 24 months (case logs): Applicant must provide documentation (case list) of provision of clinical services (50 patients) representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed.
- Will provider work solo? Yes
- Licensed Candidate: Yes, Washington license or IMLC with LOQ required in order to meet our desired start date. Applicant will need to have at least applied for the Washington license via IMLC at time of pre-app
- Required certifications (Must be from American Heart Association or American Red Cross as applicable): BLS
