Board Certification & Licensing:
- Must be Board Certified/Board Eligible in Radiation Oncology (MD or DO).
- Active Illinois (IL) medical license and IL DEA/CSR are required.

Schedule & Call Expectations:
- Monday – Friday, 8:00 AM – 4:30 PM.
- Some potential calls depending on the confirmed schedule and credentialing.

Travel & Reimbursement:
- Driving Distance: Round-trip mileage & lodging covered.
- Flights: Indicate necessity on the Stratum form if applicable

Reference Requirements:
- At least two professional references must be completed at the time of presentation.
- References should be from recent colleagues or medical directors who can attest to the provider’s skills, experience, and professionalism.
