Qualifications:
MD or DO Degree.
Valid Illinois Medical License (or ability to obtain IL Medical License).
Valid Illinois Controlled Substances License (or ability to obtain IL License).
Board eligible or Board Certified-PM&R Residency.
Compensation and Benefits:
