Requirements:
- ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
- Board Eligible (within 5 years of residency) or Board Certified in OB/GYN - [REQUIRED]
- CA license (or willing to obtain) - [REQUIRED]
- OB Hospitalist work experience - [PREFERRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- Is provider interested in full-time or part-time employment? (note in "Additional Submission Details" field) - [REQUIRED]
