• Locum Tenens – 1099 Contract
• Assignment Duration: 13 or 26 weeks
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM (5x8 shifts)
• Call: Office call only after hours and weekends
• Practice Setting: Ambulatory clinic

Job Responsibilities
• Support staff available including secretary and LPN during clinic days
• Coverage required at ambulatory clinic and Clifton Springs OR space
• Patient acuity: Ambulatory patients only
Qualifications
• Board Eligible or Board Certified
• Minimum 1–2 years of experience post fellowship
• Active New York Medical License (Required)
• Current BLS Certification
• EPIC EMR experience required
Working Hours
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM (5x8 shifts)
• Call: Office call only after hours and weekends

• Radiofrequency Ablation (Cervical, Thoracic, Lumbar)
• Nerve Blocks (various anatomical locations)
• Trigger Point Injections
• Sacroiliac Joint Injections
• Spinal Cord Stimulator Trials
• Genicular Nerve Blocks and Genicular RFA
• Intercostal Nerve Blocks
• Piriformis Injections
