Requirements:

• Board Certified in Pediatrics - [REQUIRED]
• TRULY Board Eligible (within 5 years of fellowship working toward BC) or Board Certified in Neonatal-Perinatal Medicine - [REQUIRED]
• KS license (or willing to obtain) - [REQUIRED]
• Fellowship-trained in Neonatal-Perinatal Medicine (to be completed prior to start) - [REQUIRED]
• Clean malpractice and license history - [HIGHLY PREFERRED]
• Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
• Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
• DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) [REQUIRED] * no exemptions
