Lead clinical excellence through provider mentorship, quality improvement, performance management, regulatory compliance, and evidence-based care while maintaining a reduced patient panel. Job Responsibilities

β€’ Provide clinical supervision, mentorship, and ongoing support to physicians and APPs across Massachusetts.
β€’ Oversee provider performance, documentation quality, patient outcomes, productivity, and clinical safety standards.
β€’ Lead quality improvement initiatives, chart reviews, case audits, and clinical best-practice development.
β€’ Collaborate with multidisciplinary teams on program expansion, clinical innovation, onboarding, and provider education.
β€’ Provide direct home-based primary care, including assessments, diagnosis, treatment planning, medication management, and patient education.

Qualifications

β€’ MD or DO from an accredited U.S. medical school.
β€’ Completion of an accredited Family Medicine or Internal Medicine residency.
β€’ Board Certified in Family Medicine or Internal Medicine.
β€’ Current and unrestricted Massachusetts medical license with prescriptive authority.
β€’ Minimum 5–7 years of clinical experience.
β€’ Demonstrated medical leadership experience.
β€’ Home-based care, primary care, geriatrics, or complex disease management experience preferred.
β€’ Geriatrics training or palliative care experience is a plus.
β€’ Strong clinical judgment, communication, mentorship, and leadership skills.
β€’ Proficiency with EMR/EHR systems and clinical documentation standards.
