Lead clinical excellence through provider mentorship, quality improvement, performance management, regulatory compliance, and evidence-based care while maintaining a reduced patient panel. Job Responsibilities
• Oversee provider performance, documentation quality, patient outcomes, productivity, and clinical safety standards.
• Lead quality improvement initiatives, chart reviews, case audits, and clinical best-practice development.
• Collaborate with multidisciplinary teams on program expansion, clinical innovation, onboarding, and provider education.
• Provide direct home-based primary care, including assessments, diagnosis, treatment planning, medication management, and patient education.
Qualifications
• Completion of an accredited Family Medicine or Internal Medicine residency.
• Board Certified in Family Medicine or Internal Medicine.
• Current and unrestricted Massachusetts medical license with prescriptive authority.
• Minimum 5–7 years of clinical experience.
• Demonstrated medical leadership experience.
• Home-based care, primary care, geriatrics, or complex disease management experience preferred.
• Geriatrics training or palliative care experience is a plus.
• Strong clinical judgment, communication, mentorship, and leadership skills.
• Proficiency with EMR/EHR systems and clinical documentation standards.
Working Hours
