Physician – Post-Acute & Long-Term Care / Medical Director

Key Responsibilities

Medical Director & Leadership Duties

• Serve as Medical Director for assigned skilled nursing facilities.
• Collaborate with the Regional Medical Director to implement clinical programs, quality initiatives, and best practices.
• Provide clinical leadership, mentorship, and support to healthcare providers and facility staff.
• Partner with facility administrators and interdisciplinary teams to improve patient outcomes and operational performance.
• Participate in quality assurance, performance improvement, and regulatory compliance initiatives.
• Support survey readiness and ensure adherence to federal, state, and facility regulations.
• Attend and contribute to interdisciplinary care conferences and leadership meetings.

Clinical Responsibilities

• Conduct comprehensive patient evaluations and ongoing medical management in post-acute and long-term care settings.
• Diagnose and treat acute and chronic medical conditions.
• Order, review, and interpret laboratory tests, imaging studies, and diagnostic evaluations.
• Develop individualized, evidence-based treatment plans.
• Prescribe medications and therapies in accordance with clinical guidelines.
• Complete timely and accurate medical documentation, including histories, physical examinations, progress notes, and care plans.
• Review and sign laboratory results, clinical documentation, and regulatory forms within established timeframes.
• Educate patients, families, caregivers, and facility staff regarding treatment plans and disease management.
• Collaborate with nursing staff, rehabilitation teams, specialists, and other healthcare professionals to ensure coordinated patient care.

Qualifications

Required

• Doctor of Medicine (MD), Doctor of Osteopathic Medicine (DO), or MBBS degree.
• U.S. Citizen or authorized to work in the United States.
• Eligible for an unrestricted medical license in Montana (MT).
• DEA registration and National Provider Identifier (NPI), or ability to obtain.
• Eligible for participation in Medicare and Medicaid programs.

Board Certification

Board Certified or Board Eligible in one of the following specialties:

• Internal Medicine
• Family Medicine
• Geriatric Medicine
• General Practice
• Other relevant specialties may be considered
