Job Responsibilities
β’ Supervise PCPs and Advanced Practice Providers
β’ Lead Quality Assurance Performance Improvement (QAPI) initiatives
β’ Develop and maintain medical policies, protocols, and standards
β’ Conduct patient assessments, physicals, and care planning
β’ Collaborate with interdisciplinary teams and participate in care conferences
β’ Educate patients and families on chronic disease management
β’ Support program growth, operations, and community engagement
Qualifications
β’ Geriatrics Fellowship or Certification (Preferred)
β’ Indiana medical license or willingness to obtain (Required)
β’ DEA, CDS, and BLS certifications (Required)
β’ Minimum 1 year experience with elderly/frail population (Required)
β’ Prior leadership/supervisory experience (Required)
β’ Strong clinical and organizational skills
