Lead an innovative PACE (Program of All-Inclusive Care for the Elderly) as a Medical Director, delivering comprehensive, community-based care to seniors with chronic needs. This leadership role blends clinical care with program oversight, offering a rewarding opportunity to improve quality of life while working in a collaborative, mission-driven environment—no weekends or call required.
Job Responsibilities
• Supervise PCPs and Advanced Practice Providers
• Lead Quality Assurance Performance Improvement (QAPI) initiatives
• Develop and maintain medical policies, protocols, and standards
• Conduct patient assessments, physicals, and care planning
• Collaborate with interdisciplinary teams and participate in care conferences
• Educate patients and families on chronic disease management
• Support program growth, operations, and community engagement
Qualifications
• Geriatrics Fellowship or Certification (Preferred)
• Indiana medical license or willingness to obtain (Required)
• DEA, CDS, and BLS certifications (Required)
• Minimum 1 year experience with elderly/frail population (Required)
• Prior leadership/supervisory experience (Required)
• Strong clinical and organizational skills
