Lead an innovative Program of All-Inclusive Care for the Elderly (PACE) as a Medical Director, providing comprehensive, community-based care for seniors with chronic needs. This leadership role combines clinical excellence with program oversight, allowing you to make a meaningful impact by helping elderly patients maintain independence while receiving coordinated, high-quality care.
Job Responsibilities
• Conduct history & physical exams, reassessments, and develop treatment plans
• Supervise PCPs and Advanced Practice Providers
• Lead Quality Assurance Performance Improvement (QAPI) initiatives
• Collaborate with interdisciplinary teams for comprehensive care planning
• Participate in intake, assessments, and care coordination decisions
• Educate patients and families on chronic conditions and care plans
• Oversee medical policies, procedures, and clinical standards
• Participate in on-call rotation as needed
Qualifications
• Geriatrics experience or certification (Preferred)
• Active Massachusetts medical license (Required)
• DEA and CDS certifications (Required)
• Prior leadership/supervisory experience (Required)
• Minimum 1 year experience working with elderly/frail population (Required)
• Strong background in chronic disease management and patient care
