Outstanding opportunity for a Board-Certified Internal Medicine or Family Medicine Physician to serve as a Medical Director within a well-established PACE program in Massachusetts. This leadership role focuses on delivering comprehensive, community-based care to frail and elderly adults who qualify for nursing home–level care while maintaining independence at home whenever possible.

The Medical Director provides clinical oversight, supervises Primary Care Providers and APPs, and leads Quality Assurance and Performance Improvement initiatives. This is a collaborative, interdisciplinary environment designed to deliver the full continuum of medical and supportive services to seniors with complex chronic conditions.

Ideal for physicians passionate about geriatric medicine, interdisciplinary care, and clinical leadership in an innovative healthcare model.

Job Responsibilities

β€’ Provide medical direction and oversight of participant care
β€’ Conduct history & physical exams, reassessments, and treatment planning
β€’ Supervise PCPs and Advanced Practice Providers
β€’ Lead Quality Assurance Performance Improvement (QAPI) initiatives
β€’ Develop and maintain medical policies, standards, and protocols
β€’ Participate in interdisciplinary team meetings and family conferences
β€’ Oversee chronic disease management for frail elderly participants
β€’ Order and interpret diagnostic tests and imaging
β€’ Participate in provider on-call rotation as required

Qualifications

β€’ Board Certified in Internal Medicine or Family Medicine (Required)
β€’ Board Certification in Geriatrics (Preferred)
β€’ Active Massachusetts Medical License (Required)
β€’ DEA Registration (Required)
β€’ Minimum 1 year experience working with frail or elderly population (Required)
β€’ Previous supervisory or leadership experience (Required)
β€’ Recent NPDB self-query report
β€’ Strong background in chronic disease management
