Schedule: Monday–Friday, daytime clinical hours (approx. 8:00 AM – 5:00 PM)

Compensation: $240,000–$270,000 annually (including performance bonus)

Qualifications
Board Certified in Internal Medicine or Family Practice - Required
Board Certification in Geriatrics - Preferred
CDS Certification and DEA Registration - Required
Previous Supervisory experience - Required
Minimum of 1 year of documented experience working with a frail or elderly population - Required
Effective skills in physical assessment and chronic disease management for frail older adults - Required
Able to manage changing priorities according to participant needs - Required
