Job description

Job description Primary Care Physician – PACE / Geriatric Medicine (Per Diem)

Position Overview

Seeking a Primary Care Physician to provide comprehensive, community-based healthcare services to vulnerable seniors through a Program of All-Inclusive Care for the Elderly (PACE).

This per-diem position offers a flexible weekday schedule based on coverage needs, allowing physicians to deliver holistic, value-based care while supporting older adults in maintaining their independence and remaining in their homes and communities.

The physician will collaborate with an interdisciplinary team (IDT) to perform comprehensive patient assessments, develop personalized care plans, and coordinate medical services.

With a smaller patient caseload than traditional primary care settings, physicians can build meaningful, long-term patient relationships and focus on individualized healthcare needs.
