About the Opportunity

As a Primary Care Physician, you'll provide comprehensive, relationship-based care to an established panel of senior patients.

Rather than focusing on high patient volumes, this practice emphasizes preventive medicine, chronic disease management, and personalized care.

You'll have the time to understand your patients' medical needs, build meaningful relationships, and address the social and lifestyle factors that influence their overall health.

Key Responsibilities

• Provide comprehensive primary care to an established senior patient population.

• See approximately 12–15 patients per day.

• Conduct wellness visits, physical examinations, and preventive screenings.

• Diagnose, treat, and manage acute and chronic medical conditions.

• Develop individualized treatment plans based on each patient's needs.

• Coordinate care with nurses, behavioral health professionals, and other clinical team members.

• Educate patients and families regarding treatment options, preventive care, and chronic disease management.

• Maintain accurate and timely medical documentation.

• Support a collaborative, patient-centered approach to care.

Qualifications

Required

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).

• Board Certified or Board Eligible in:

  • Family Medicine

  • Internal Medicine

  • Geriatric Medicine

• Active, unrestricted Louisiana medical license or willingness to obtain one.

• Current, unrestricted DEA registration.

• Commitment to delivering compassionate, high-quality patient care.

• Strong communication and clinical decision-making skills.

Preferred

• Experience caring for geriatric or medically complex patients.

• Background in outpatient primary care.

• Interest in preventive medicine and chronic disease management.

• Experience working within a collaborative, team-based care model.
