Job Summary:

We are seeking a dedicated and compassionate Post Acute Primary Care Physician (MD/DO) to join our growing post-acute care team in Jacksonville, FL. In this role, you will perform daily rounds at designated Skilled Nursing Facilities (SNFs), ensuring high-quality transitional and long-term care for our residents. You will manage complex clinical cases, admit new patients, and provide vital clinical oversight to a collaborative team of Advanced Practice Providers (APPs).

Schedule:

• Full-time: 5 days per week
  

• Hours: Monday to Friday, 8:00 AM – 5:00 PM
  

Key Responsibilities:

• Conduct daily clinical rounds across assigned Skilled Nursing Facilities (SNFs) in the Jacksonville area.
  

• Evaluate and examine all newly admitted patients to establish comprehensive care plans.
  

• Directly manage higher-risk, medically complex patients requiring intensive monitoring.
  

• Provide clinical supervision, mentorship, and chart sign-offs for Advanced Practice Providers (APPs).
  

Qualifications & Requirements:

• Board Certified (BC) or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics (Board Certification is NOT a requirement).
  

• Active Florida Medical License in good standing.
  

• ACN (Area of Critical Need) license holders are accepted and welcome to apply.
  

• Strong communication skills and a passion for geriatrics/post-acute medicine.
  

Compensation & Benefits:

• Base Salary: $225,000 – $245,000 per year
  

• Time Off: 2 weeks (14 days) of Paid Time Off (PTO)
