Overview:
* Join an innovative Value-Based Care Clinic providing evidence-based, population-centered Internal Medicine care
* Serve as core faculty for an Internal Medicine Health Equity Track
* Combine direct Primary Care with resident education, population health, health equity and value-based care
* Supervise and provide real-time clinical teaching to Internal Medicine residents
* Teach Primary Care, population health, value-based care and health equity-focused clinical practice
* Mentor residents in quality improvement, health equity research and scholarly projects
* Participate in journal clubs, lectures, case conferences and resident workshops
* Collaborate with residency and program leadership to develop and improve curriculum aligned with ACGME competencies
* Deliver comprehensive adult Internal Medicine care within a Value-Based Care Clinic
* Utilize population health tools, risk stratification and care coordination strategies to improve outcomes and reduce healthcare disparities
* Identify and address social and non-medical drivers of health
* Work collaboratively with care managers, pharmacists, social workers, behavioral health specialists and community partners
* Participate in quality improvement initiatives and use performance/outcomes data to improve clinical care
* Collaborate with Graduate Medical Education personnel across affiliated academic institutions
* No call responsibilities
* No weekend responsibilities
* Completion of an ACGME- or AMA-approved residency required
* Board Certification or Board Eligibility in Internal Medicine required
* Texas medical license or willingness to obtain required
* Interest in resident education and academic medicine required
* Medical education, health equity, population health or value-based care experience highly preferred
* Clean malpractice and licensing history highly preferred
