Requirements

MD/DO
-
BLS certification
-
Must demonstrate inpatient or consultative clinical activity for at least 30 patients within the past 24 months, or have completed an affiliated accredited residency, special clinical fellowship, or research within the past 12 months.

Applicants who recently completed residency must provide residency case logs.

Other applicants must provide a hospital quality profile documenting clinical activity over the past 24 months.
