Lead Whole-Person Care for Older Adults
A mission-driven healthcare organization is seeking a Medical Director to provide clinical leadership and hands-on medical care within a comprehensive Program of All-Inclusive Care for the Elderly (PACE).
This is an excellent opportunity for a Board-Certified or Board-Eligible Family Medicine, Internal Medicine, or Hospice & Palliative Medicine physician who enjoys caring for medically complex older adults and wants to combine direct patient care with meaningful clinical leadership.
The Medical Director works closely with an interdisciplinary team of physicians, advanced practice providers, nurses, social workers, therapists, and other healthcare professionals to deliver coordinated, person-centered care that helps older adults remain safely in their communities.
Position Highlights

Job Responsibilities
Provide comprehensive medical evaluation, diagnosis, treatment and prescribing
Serve as a medical resource for the interdisciplinary care team
Develop and review individualized participant care plans
Provide clinical guidance regarding disease processes, prognosis and treatment
Oversee assigned advanced practice providers and support provider education
Evaluate participants in inpatient and residential settings
Provide palliative and hospice-related care, including symptom and medication management
Participate in QAPI initiatives and utilization review
Support appropriate healthcare resource utilization
Maintain accurate clinical documentation and coding
Collaborate with hospitals, community physicians and other healthcare organizations
Educate healthcare professionals and community partners about PACE services
Maintain appropriate hospital privileges
Participate in after-hours/on-call medical coverage
Qualifications
Required:
MD or DO from an accredited medical school
Minimum 3 years of physician practice experience
Current Colorado physician license
Board Certified or Board Eligible in:
Family Medicine
Internal Medicine
Hospice & Palliative Medicine
Current CPR certification
Valid driver's license
Current automobile insurance
Ability to lift at least 30 pounds
