Join a mission-driven primary care practice serving older adults in underserved communities. Unhurried 20-minute visits, a capped panel, and a care team that helps address housing, nutrition, and transportation needs, so you can treat the whole patient.
HIGHLIGHTS
- $250K"$290K+ base + quarterly performance bonus
- Outpatient adult primary care " older adult focus
- Panel capped at 750
- 15"18 patients per day (~20 min visits)
- One remote/telehealth day per week after ramp-up
- Light call: 2 evenings/month + 1 weekend/quarter
- Integrated care team (EMTs, social support specialists)
- 15% reduction in hospital readmissions
WHAT YOU'LL DO
- Provide comprehensive adult primary care, in clinic and via telehealth
- Manage complex chronic conditions in older adults
- Lead daily team huddles on care plans and priorities
- Partner with care team members on integrated patient plans
- Manage patient communication and follow-ups
- Coordinate with emergency departments on continuity of care
- Document in Athena EHR
REQUIREMENTS
- MD or DO, BC/BE in Internal Medicine
- Active Massachusetts license, or ability to obtain before start
- Strong evidence-based primary care skills
- Comfort with EHR and digital health tools
- Ability to connect with patients from diverse backgrounds
- Strong communication and documentation skills
PREFERRED
- Experience with older adults in complex social settings
- Spanish proficiency
- Telehealth or remote patient monitoring experience
LOCATION " WORCESTER, MA
- New England's second-largest city, with a lower cost of living than Boston
- Home to UMass Chan Medical School and a strong academic medical community
- Diverse, multilingual patient population
- Parks, lakes, and quick access to hiking, skiing, and the coast
- About an hour to Boston by car or commuter rail
