Job Responsibilities

The Primary Care Physician will provide comprehensive outpatient care to adult patients, including:

Preventive medicine and health maintenance
Acute care evaluation and treatment
Diagnosis and management of chronic conditions
Hypertension and diabetes management
Dyslipidemia and obesity management
Cardiovascular disease prevention and risk reduction
Individualized treatment planning
Long-term patient relationship management
Care coordination with cardiologists and other specialists
Clinical guidance to members of the care team
Participation in quality improvement and workflow development

Qualifications

MD or DO from an accredited medical school
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Active, unrestricted New York State medical license
Active DEA registration
Strong clinical judgment and diagnostic skills
Experience managing chronic medical conditions
Excellent communication and interpersonal skills
Comfortable working with EHR systems and modern healthcare technology
Interest in preventive medicine and cardiovascular risk management
2+ years of outpatient or private practice experience preferred
