Sonoma, California, Hospice Physician Job ~ Greater San Francisco Bay Area - Hospice Fellowship Not Required
RecruitWell is partnering with a very well-established, hospice & palliative service provider searching for a qualified physician to join their team.
Position Overview
Full-Time Position
Schedule:
Monday - Friday
8am-5pm
On-Site: 2 days per week
Attend interdisciplinary team meetings
Discuss patient's care, set goals, review progress, and coordinate services
Remote: 3 days per week
Charting
Answer Calls
Collaborate with Primary Care Providers
Afterhours Call: Wednesday Nights(5pm-8:30am)
Provide physician oversight for hospice patients via telephone and telemedicine
20 Hours Per Week Focused around Hospice
10 Hours Per Week focused around General Inpatient Care Program (GIP)
10 Hours Per Week focused around Palliative Care
Manage complex symptoms and adjust medications to maximize comfort and quality of life
Respond to admission and clinical consultation calls from nurses and admission staff
Collaborate closely with hospice nurses, nurse practitioners, social workers, chaplains, and other interdisciplinary team members
EMR: EPIC
Qualifications
M.D. or D.O. degree.
Board certified or board eligible in any of the following specialties:
Hospice & Palliative Care
Internal Medicine
Family Medicine.
Open to experienced physicians and new graduates.
Active California medical license or eligibility for California licensure.
Excellent interpersonal and communication skills, with the ability to build strong relationships with patients, families, colleagues, and referring providers.

Compensation: $150-$160 per Hour
