Job description
Job description
Tri-County Heart Institute is an independent, physician-owned cardiology practice serving The Villages, Lady Lake, and Leesburg, Florida. We are adding a full-time internal medicine physician to build the primary care side of our Women's Heart Center.
Dr. Nitza Alvarez, our founder and medical director, built the Women's Heart Center because she kept seeing the same problem in this community. Women were under-recognized and under-treated for cardiovascular disease, and by the time many of them reached a cardiologist, the window where prevention would have mattered most had already closed.
The program has outgrown what cardiology alone can do. Cardiology is downstream. This role exists so that blood pressure, lipids, metabolic health, weight, sleep, and menopause-related risk are managed from the beginning, rather than after an event.
Tri-County has been practicing prevention for ten years, well before prevention became popular. This is not language we adopted recently. It is how the practice was built.

Job Responsibilities
- Provide comprehensive internal medicine care: acute visits, chronic disease management, preventive care, and coordination of specialty referrals.
- Own the modifiable risk factors that drive cardiovascular disease in this population: hypertension, lipids, diabetes and prediabetes, obesity and weight management, tobacco use, sleep, and physical activity.
- Serve as the primary care arm of the Women's Heart Center, identifying and managing cardiovascular risk in women earlier than a cardiology referral would allow, including risk associated with menopause, hypertensive disorders of pregnancy, and other sex-specific factors.
- Perform Medicare annual wellness visits and initiate advance care planning conversations.
- Participate in chronic care management and remote patient monitoring programs, working with the care coordination team.
- Coordinate directly with our cardiologists on shared patients, with in-house echocardiography, nuclear cardiology, and diagnostic testing available same-practice.
- Collaborate with and help supervise advanced practice providers as the primary care program grows.
- Maintain accurate and timely documentation, including problem list hygiene and risk-adjustment coding, and manage the clinical inbox, refills, and prior authorizations with support staff.
- Participate in quality improvement and performance measure work tied to value-based and Medicare Advantage contracts.
- Help shape the primary care program as it builds: workflows, protocols, patient education, and scope.
