POST-ACUTE CARE PHYSICIAN – HICKORY, NC

The practice is actively interviewing Board-Certified or Board-Eligible physicians trained in Internal Medicine, Family Medicine, or Geriatric Medicine.
Primary Responsibilities
- Provide medical care to patients in skilled nursing and post-acute care facilities
- Manage both acute and chronic medical conditions
- Develop individualized treatment plans and coordinate follow-up care
- Collaborate with nursing staff, facility leadership, specialists, and community providers
- Complete timely and accurate clinical documentation
