Healthcare United is seeking Board Eligible or Board Certified Internal Medicine or Family Medicine physicians for outpatient primary care opportunities in Southbury, Naugatuck, Shelton, and Oxford, Connecticut.
There are multiple openings available for physicians interested in joining an established outpatient primary care network with strong clinical support, a growing patient base, and desirable Southern Connecticut locations.
The incoming physicians will provide comprehensive adult or family medicine care in a community-based outpatient setting.
Job Responsibilities
• Evaluate and manage adult or family medicine patients
• Conduct wellness visits, sick visits, chronic disease management, and preventive care
• Develop individualized treatment plans
• Coordinate care with specialists, APPs, nurses, and clinical staff
• Maintain accurate and timely documentation in the EMR
• Support quality care, patient access, and continuity of care
• Build long-term relationships with patients and the community
Qualifications
• Active or eligible Connecticut medical license
• Outpatient primary care experience preferred
• Strong clinical assessment and communication skills
• Interest in community-based primary care
