• Seeking an experienced and mission-driven Medical Director to lead and support a provider team delivering home-based primary care across Massachusetts
• Provide clinical leadership, provider mentorship, quality oversight, and regulatory guidance
• Serve as a primary clinical resource for complex cases and diagnostic questions
• Maintain a reduced patient panel while providing direct clinical care
• Foster a culture of clinical excellence, collaboration, and continuous improvement

Job Responsibilities
• Serve as a resource for clinical questions, diagnostic guidance, and complex case consultations
• Monitor adherence to evidence-based clinical standards and treatment protocols
• Conduct ride-alongs, chart reviews, and case audits
• Oversee provider performance, productivity, documentation, patient outcomes, and safety
• Lead quality improvement initiatives focused on clinical effectiveness and patient satisfaction
• Develop workflows, best practices, and quality benchmarks
• Provide coaching, feedback, and performance evaluations
• Collaborate with physicians, PAs, NPs, specialists, behavioral health providers, and care coordinators
• Support provider onboarding, training, and continuing education
• Conduct comprehensive home-based health assessments and physical examinations
• Diagnose conditions, develop individualized care plans, and prescribe medications
• Educate patients and families regarding disease management and preventive care
• Maintain accurate EHR documentation
Qualifications
• Successful completion of Internal Medicine or Family Medicine residency
• Current Massachusetts medical license and prescriptive authority
• Board Certified in Internal Medicine or Family Medicine
• Minimum 5–7 years of clinical experience
• Demonstrated leadership experience
• Experience in home-based care, primary care, geriatrics, internal medicine, or complex disease management preferred
• Geriatrics training or palliative care experience preferred but not required
• Proficiency with EMR/EHR systems and documentation standards
