Join our Senior Primary Care Office in Venice, FL
Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a family, internal, or geriatric medicine physician to join our growing team in this established office. Grow with us as we expand our reach to help more Seniors live better, healthier lives.
You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes. Job Responsibilities
-Monday-Friday 8:00am-5:00pm
-Seeing an average of 15 patients per day. Opportunity to spend more time with patients
-Spend your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
Qualifications
-Doctor of Medicine or Osteopathic Medicine
-Board Certified/Board Eligible in family, Internal, or geriatric medicine
-Valid, unrestricted license in Florida (or must obtain)
-Current, unrestricted DEA license (or obtain)
- Two years' experience seeing patients 65 and over and/or managing patients with chronic medical conditions
Preferred Qualifications:
-Experience managing Medicare Advantage panel of patients
-Knowledge of Medicare guidelines and coverage
-Knowledge of HEDIS quality indicators
Working Hours
Outpatient only, in a value-based care model.
