Family Medicine/Internal Medicine Physician

We are seeking a dedicated physician to join our team and contribute to our mission of providing high-quality healthcare services. Our team is comprised of experienced physicians and Advanced Practice Providers (APPs) working collaboratively to ensure the best outcomes for our patients.

Responsibilities

• Manage a patient daily baseline of 16 patients, totaling 80 per week.
• Flexible schedule options: work either 5 days of 8-hour shifts or 4 days of 10-hour shifts.
• Participate in a call rotation, phone-only, once every 12 weeks. During the call week, work 3 hours on a Saturday morning with a corresponding reduction of 3 hours during the week.
• Work out of 1 or 2 locations, with a typical schedule of 3 days at one clinic and 2 at another, depending on location choice. Milford, West Haven, and Hamden are the busiest locations, with the option to work exclusively at one.
• Conduct 30-minute patient visits, with the ability to double book if necessary.
• Incorporate telehealth or Chronic Care Management (CCM) visits as a percentage of patient interactions.
• Utilize 2 patient rooms and work with a dedicated Medical Assistant (MA) who handles all pre-authorizations, vitals, and related tasks.

Requirements

• Board certified or board eligible in Internal Medicine or Family Medicine.
• Possess a valid medical license to practice in the state.
• Strong commitment to providing high-quality patient care.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a team environment.
• Proficiency with Electronic Medical Records (EMR) systems, preferably EClinical Works.
